Who drops out of CBT for chronic pain?

Wouldn’t it be wonderful if everyone we saw was ready for self management and committed to putting everything in place? Wouldn’t it be even better if we could tell who was and who wasn’t going to drop out? Then we could focus treatment on people who were ready for treatment, and help those who are … Read more... What obstacles get in the way of self managing chronic pain?

Learning to manage pain using self management strategies ain’t easy – as we know, changing habits and doing things differently is not something any of us do without a bit of effort. It’s especially difficult when life in general is full of challenges, but so much more so when pain is the problem, you have a … Read more... Bair, M., Matthias, M., Nyland, K.

Bridging the self-management gap

Recently I ran an online survey on this blog asking the question ‘Should self management include:’ and then I listed a number of options such as ‘injection therapy’, ‘medications’, ‘intermittent hands-on therapy’, ‘intermittent hands-off therapy’ and so on. My thoughts were that while the term ’self-management’ is bandied ab

How do you establish who will do well with pain management?

Some people just won’t do well with pain management.  In just the same way as a surgeon selects good candidates for surgery, so people need to be selected for self management.  Although there is some truth that getting even a little pain management is good for everyone, the cost of doing so in staff energy [...]... How To Make Tour Child a Psychological Wreck - The Playground Manual

Push the kid the whole way to the playground in their stroller.  "I have to - otherwise they'll run out in the street."Well, you're getting them ready to be adults who try to park as close to the entrance door to their health club as possible, to avoid extra walking.  You know, walking is exercise - as well as generally good prep for life self-management.  Everyone has to learn how

Self-care or medical care for low back pain: what patients want

For a while I’ve been asking what constitutes ’self management’ for chronic pain. On the one hand there are a group of people who firmly believe that regular medical treatment (including injections every three months or so) is a perfectly legitimate way to maintain a normal life. On the other hand there are people [...]... Saunders KW, Von Korff M, Pruitt SD, & Moore JE

Early management of low back pain – activity is good, early self-management is best!

It must be really difficult to be a reductionist, biomedically-oriented doctor who believes fervently in the need to ‘abolish pain’ in order for people to recover from low back pain. It must be even more difficult when the ’source of the nociception’ fails to be identified even after numerous placebo-controlled ‘diagnostic’ nerve blocks have [...]...

When to start self management for chronic pain

There’s no doubt that early self management for chronic pain is a good thing – but in our facility, we’ve always had a rather mixed feeling about introducing self management while people are still receiving medical treatment for their pain. Our experience has been that few patients are really committed to learning how to [...]... Mead, K., Theadom, A., Byron, K., & Dup

Chronic disease management – follow-up and support needs from Diabetes – is this a model for Pain?

Self management for chronic pain is not the only area in which self management has been introduced. Heart disease, obesity and diabetes are all very commonly managed with a combination of biomedical and self management strategies. While reviewing different ways to provide support for people who have newly developed coping strategies, I have [...]... Fisher, E., Brownson, C., O'Toole, M

Self-management follow-up – a focus group study

After attending a workshop on self-management run by the Ministry of Health last week, I thought it might be useful to look at some of the areas studied in other countries before New Zealand runs headlong into thinking that self-management is both inexpensive and easy. Not that I am in any way saying that [...]... Matthias, M., Bair, M., Nyland, K., Huffman, M., Stubbs, D., Damush, T.,
